(A complete clinical nutritional and dietary counseling is given along with a general list of changes to be made in the diet and lifestyle. The plan includes 4 sessions in total which are spaced apart by 2 weeks for 2 months from enrolling in the plan. JHMC^® Food diary is provided as a complement to reinforce the patient to become committed to the dietary changes and the diary should be brought for the follow-up visits to review your actions).
